Trusted nursing care in your home

Our in-home nursing services support NDIS participants with complex, high, or ongoing health needs to live well at home and in the community. We work collaboratively with participants, families, support coordinators, plan managers, and healthcare professionals to ensure care is seamless, responsive, and aligned with each participant’s NDIS plan.

All supports are delivered in line with NDIS requirements, best-practice clinical standards, and comprehensive risk and safety frameworks — giving families and coordinators confidence that care is both clinically sound and genuinely compassionate.

Our NDIS nursing Services

High-intensity daily nursing supports

We provide high-intensity nursing care for participants with complex medical conditions requiring frequent monitoring, skilled clinical judgement, and timely interventions to reduce risk and avoid unnecessary hospital admissions.

Wound care and pressure injury management

Our nurses deliver evidence-based wound and pressure injury care focused on infection prevention, skin integrity, and optimal healing outcomes. Education is provided to participants and carers to support ongoing care between visits.

Medication management

We support participants with safe medication administration, monitoring for side effects, and ensuring medications are taken correctly and consistently, with clear clinical documentation and communication.

Enteral feeding and nutritional support

Our nurses provide specialised PEG and nasogastric feeding support, including monitoring tolerance, nutritional intake, and overall well-being, while working closely with dietitians and allied health professionals.

Continence and catheter care

We deliver respectful, individualised continence care, including catheter management and toileting programs, with a strong focus on comfort, dignity, and independence. We also provide continence assessments as required.

Tracheostomy and ventilator care

Holistic Disability Solutions offers advanced respiratory nursing support, including tracheostomy care, suctioning, and ventilator management, delivered safely in the home by trained, competent and highly trained nurses.

Palliative and end-of-life nursing care

We provide compassionate palliative and end-of-life nursing care that prioritises comfort, symptom management, dignity, and emotional support for participants and their families during life-limiting illness. We provide compassion and family involvement in the entire process. We want to work with you, the family and the participants' needs.
